All clients must provide the following identification in order to apply for assistance:
- Current Picture ID
- Social Security cards for each household member
- Proof of address
- Documentation of assistance needed (utility cutoff notice, court copy of eviction containing Judge's determination, written prescription)
SPAN's services are directed to the household.
All individuals, whether related or not, living at the same address, will be considered as one (1) household.
